I'm Lance Granum RN IPAC. I’m the very proud Director of Nursing at Veritas Advocacy. We're a veteran-owned and operated organization, and we specialize in supporting our brave military veterans who have also served this great country. We understand the unique challenges that our veterans face and are committed to providing them with the highest level of care and support.
Our team is made up of independent nurse advocates who are as dedicated as I am to working directly for patients and their families. We're not tied to any insurance, hospitals, nursing homes, rehabilitation facilities, or post-acute care facilities. Instead, we're here for you and your loved ones, providing guidance and advocacy every step of the way.
Navigating the health journey from wellness to illness can be challenging, especially when it involves transitions from acute care hospitals to rehabilitation, home health, long-term care, assisted living, or skilled nursing. That's where we come in. We're here to ensure that no one falls through the cracks during these transitions.
We take the time to get to know you and your health inside and out. We conduct thorough assessments and evaluations, and coordinate your medical records to ensure that any facility you move to understands your needs as well as we do. This not only ensures your safety but also allows us to provide you with the best possible care.
From coordinating care and planning, to reviewing medications, coordinating with your pharmacy, and even developing plans to help you stay in your home and maintain your independence if that's the safest option for you, we're here to help.
